The primary goal of Cardiac Rehabilitation is to enable the participant to achieve his/her optimal physical, psychological, and social functioning through exercise training and lifestyle change. Cardiovascular Disease & Diabetes Prevention

The CMMC Cardiovascular Disease & Diabetes Prevention Program is an affordable, one-year program. Meetings cover topics about a healthy lifestyle program that focuses on diet, exercise, and barriers that relate to achieving a healthy diet and exercise plan. Active participant have access to the CMMC Fitness Center to assist them with developing an exercise program. A daily log of exercise and food intake will be expected of all participants.

Patient must be

  1. Age 18 years or over AND
  2. Overweight/obese BMI (BMI ≥ 25 kg/m2 or if Asian, BMI ≥ 23 kg/m2) AND
  3. Have one or more of the risk factors listed:
    1. Elevated blood glucose level of 75-gram oral glucose tolerance test (OGTT) with 2-hour plasma glucose is 140-199 mg/dL, fasting plasma glucose (FPG) is 100-125 mg/dL, or A1C 5.7-6.4%
    2. Score 9 or higher on CDC prediabetes screening test.
    3. History of gestational diabetes
    4. Elevated blood pressure level of ≥130/80 mmHg OR treatment
    5. Abnormal lipid levels of HDL <50 mg/dL for women or <40 mg/dL for men, LDL >130 mg/dL, or triglycerides >150 mg/dL OR treatment
    6. Given birth to a baby weighing >9 pounds

CMMC's Care Coordination Dept provides an additional level of care for patients and their caregivers so that they can have an extra hand up in managing their chronic condition. We offer support wherever we can to make the transition into managing a long term health issue as helpful as possible. Central to our mission is the quality improvement initiative of incorporating community based care coordination designed to help patients achieve better health, better health care and lower costs.
 
Caregivers of Dementia Support Group The Caregivers of Dementia Support Group's purpose is to give support, information, skills, venting and coaching for people that are caring for or have a person in their life that has dementia.  The coaching is to help people to better communicate with and support people that have cognitive dysfunction or memory problems.  Support is available for loved ones with dementia during support group gatherings.

Clinical Nutrition Department A registered dietitian nutritionist/ ACE certified health coach provides Medical Nutrition Therapy and nutrition education/counseling through individual visits and/or group classes. The NutritionFit program, in particular, focuses on helping to reach and maintain sustained weight loss. With a referral from a provider; many insurance plans cover these sessions.

Community Paramedic Program

Community Integrated Health:

Community Integrated Health (CIH) is a newer approach to healthcare where paramedics and EMTs (emergency medical technicians) use their skills in a larger role to address community health needs. A CIH Provider (CIHP) will connect patients to appropriate community resources that better serve patients than the 911 system. These might include mental health and follow-up care for chronic diseases such as congestive heart failure, diabetes, COPD, and asthma. CIH provides preventative healthcare, home safety evaluations, and addresses medical needs in the home to reduce hospital transports for non-emergent care needs. Central Montana Medical Center is embracing the CIH movement as a way to improve access to healthcare, enhance community-based preventative care, and provide additional opportunities for EMTs and paramedics to care for their communities. Referrals can come from anyone who identifies that there may be a need, not just a physician - once the referral is made the patient will be contacted for consent to the program. Diabetes Education Diabetes Education at CMMC is an outpatient program that was designed to meet the highest standards for diabetes education. Since 2005, Central Montana Medical Center has been recognized as meeting the National Standards for Diabetes Patient Education, as developed and reviewed under the auspices of the American Association of Diabetes Educators. Recognition is a formal acknowledgement of excellence. The program at CMMC includes group and/or individual instruction to help persons with diabetes learn to manage their disease in an atmosphere of compassion and support. Education focuses on incorporating healthy eating and physical activity, monitoring blood sugar to avoid the risk of complications, problem solving and adjusting emotionally to diabetes. Diabetes education also offers a free support group that meets on the second Thursday of every month in the office space outside of Seiden Drug that is led by Linda Allen and Emily Heggem. At CMMC we use a team approach to help guide you through your treatment and help with any fears, issues, and problems you encounter along the way. Our team includes your personal physician, a certified diabetes educator, a registered dietitian, and a fitness specialist.

Regular activity is important for overall fitness, weight management, and blood sugar control. With your health care provider’s approval, Debbie can help you develop an exercise plan that is right for you and one you will enjoy doing. The Diabetes Self Management Education is covered by Medicare, Medicaid and by many insurance plans. Please contact your plan to find out more about your coverage. Self-pay is also accepted. A referral from your Primary Care Physician is required.

In an emergency, you want medical care as quickly as possible.  Central Montana Medical Center’s Emergency Department is staffed with dedicated physicians and nurses, with access to the Operating Room (OR), lab, imaging, and other services 24 hours a day, 365 days a year.

We are a Level 4 Trauma Facility

Central Montana Medical Center has been trauma designated since 2009.  This designation from the Montana Department of Public Health and Human Services means that CMMC has met all criteria in trauma care with services, equipment and staff training to receive designation by the state of Montana as a Level Four Community Trauma Facility.

The Emergency Department sees a large variety of patients.

COVID-19 VACCINATION: "Vaccination against COVID-19 is one of the many tools we have at our disposal to protect our employees.  CMMC does not mandate receipt of the vaccination, however, employees are strongly recommended to receive it.  COVID vaccinations are available to all CMMC employees upon request.  The vaccination is a 2-dose series, to be given approximately 28 days apart.  There are minor side effects anticipated with this new vaccine, so it is important that any staff member wanting to receive the shot(s) consult with their Department Manager to coordinate a date/time that will be conductive to their department.  It is also quite important for staff to keep their scheduled appointment as there are many restrictions and guidelines that must be followed by those administering the vaccine.  Please call the CMMC Occupational Health department if you have any questions, or would like additional information regarding this wonderful benefit. COVID EXPOSURE "All Employees have a Duty to Report the following:
  • Infections Disease Illness Symptoms on scheduled work days
  • External Exposure to COVID(+) or Rule-out person
  • If testing in another facility for CoVID due to exposure, surveillance testing does not need to be reported
  • Possible Internal Exposure to CoVID
  • CoVID Testing done in another facility
  • Requested Public Health Quarantine
Occupational Health will do any internal investigation and make work restriction determinations for any of the above.  If testing is required, Occupational Health will provide that testing for the employee.

Tuberculosis Screening

Pre-employment PPDs are required for all new employees before they can begin work at CMMC. One-step PPDs are needed for those individuals who can provide written documentation of TB testing in the previous 12 months prior to employment. A Two-step PPD is required if no previous testing has been completed in the last year. PPDs must be read by a physician or nurse within 48-72 hours after placement. You MUST have the PPD read to complete the screening. Having it placed and not read invalidates the PPD and it must be repeated. Only those who complete the reading are considered compliant with the TB screening requirement.

Annual Tuberculosis Signs & Symptoms Questionnaire

All employees must complete the Annual Tuberculosis Signs and Symptoms questionnaire each year for the duration of employment/placement with CMMC. This requirement must be completed electronically and submitted in order for employees to receive their annual evaluation. Once you submit your questionnaire, it will be reviewed by the Infection Preventionist and the EH nurse. If your questionnaire highlights symptoms of concern, the nurse will contact you. Otherwise, the form information will be approved by the nurse and filed in your CMMC Health Services record. and in the Human Resources. record making you compliant with the annual TB screening requirement. Human Resources will then be notified you have met this portion of compliance for your annual evaluation.

Vaccinations and Immunity

Vaccinations and Immunity For the protection of patients, staff and visitors, all employees and volunteers must have written record of immunization to specified illnesses in order to volunteer or be employed at CMMC. Written documentation of lab- identified immunity is also permissable. The following is a list of illnesses for which vaccination is required:
  • Measles / Mumps / Rubella
  • Varicella (chicken pox)
  • Tetanus / diptheria / pertussis
If record of vaccinations cannot be obtained, CMMC will provide titer testing to identify immunity of MMR and varicella, at no charge to the employee. Titer results must be completed prior to beginning employment.
Tetanus / diptheria / pertussis (Tdap):
If an employee has not received the adult booster vaccination of Tdap, CMMC will provide this vaccination free of charge to the employee.
Hepatitis B:
Hepatitis B is a serious infection that is easily spread through contact with the blood or body fluids of an infected person. Hepatitis B vaccine can prevent Hepatitis B and the serious consequences of the infection. It is highly recommended for any employee that is occupationally exposed to blood or other potentially infectious materials, including the risk of needle sticks. CMMC provides this vaccination series free of charge to all employees.
Annual Flu Vaccination Program
In a continuing effort to protect our patients as well as the employees, students and volunteers that make our organization great, influenza vaccination is strongly encouraged. If an employee, student or volunteer chooses not to receive the annual influenza vaccination, they must wear a respiratory mask during the designated flu season as determined by the CMMC Infection Prevention Physician. Please review the Influenza Vaccination policy EMP-HLT 101 located on the Intranet under Hospital Policies and Procedures for the full details. CMMC begins vaccination in October of each year. The vaccine is available for employees, students and volunteers at the annual Flu Clinics, held in conjunction with the Annual In-service, which is mandatory for all employees. Vaccination is also available on a walk-in basis during the standard CMMC Employee Health office hours. The influenza vaccine is available to these groups until the end of the traditional flu season, which is usually marked as April 1st of each year.

  • Get the flu shot!
  • Wash your hands
  • Cover your nose and mouth with a tissue when you sneeze or cough
  • Avoid touching your eyes, nose, and mouth
  • If you are sick stay home
  • If your doctor prescribes flu antiviral drugs, make sure to take them

Annual Employee Health Wellness Screening

CMMC provides an annual health screening for their employees, and spouses, on the insurance program as part of a wellness benefit to encourage their employees to focus on maintaining a healthy lifestyle. CMMC believes the best way to protect and maximize your health is to start with a health screening. Health Screenings are important for early detection of disease or other possible health problems. The Health Screening is held on campus and includes a health risk assessment, vital signs, height and weight, body mass index and lab testing. The lab tests covered include: Complete Blood Count, Complete Metabolic Panel, Hemaglobin A1C, Thyroid test, Cotinine and Cholesterol levels. CMMC male employees over the age of 50, will also have a prostate level checked. Following the screening, employees receive a report that reviews all of their results and includes annual result trending if the employee participates every year.

You can request copies of your health records on file with CMMC Health Services by completing the Request for Records / Signed Consent to Release Records form. The request form and consent can be faxed to the Occupational  Health Services office at 406-535-6607. Please allow two business days for your request to be processed. Fitness Center The CMMC Fitness Center is a medically monitored health and wellness center featuring an array of programs and equipment designed to meet the conditioning needs of community members regardless of age, athletic ability, or chronic disease process. Our equipment includes free weights, Cybex weight stations, treadmills, Nusteps, Versa Climber, stationary bike, Airdyne bike, rower, Stairmaster, Ergometers, exercise balls, and much more! Our center includes:

  • Circuit training; 30 seconds of exercise with 15 seconds of rest.  Stations change monthly.

Home Health Care Services Home Care provides multiple services supervised by your physician that enable you to stay home while recovering from an illness or injury or when suffering from a long term or terminal illness. The Home Health Program has been offering comprehensive, quality home care services since 1984. Those services available through CMMC Home Health are:
  • Nurse: Visits to provide such services as Wound Care, Ostomy Care, Diabetes Education, medication education, assessment & education regarding the disease process and steps to recovery, patient & Family Education and Referrals to appropriate Therapies. Nursing services are available 7 days per week, from 8:00 am to 4:30 pm.
  • Certified Nurse Aide: Assistance with bathing, medication reminders and other personal care needs.
  • Physical Therapy: Treatments and exercise programs to facilitate mobility, transfers and safety in the home.
  • Occupational Therapy: Provides retraining in vocational and leisure activities, activities of daily living, including toileting, dressing, self care and use of specialty equipment.
  • Speech/ Language Pathology: Provides therapy for swallowing, speech and cognitive difficulties.
  • Registered Dietician: Offers dietary and nutritional counseling.
Medicare, Medicaid, Private Insurance, HMO, Workman's Compensation and VA cover most Home Health services. A Provider must order all services.

Private Duty Nursing & Aide Services

Services are provided in the home by a licensed nurse for those persons who are not eligible for Home Health through Medicare, Medicaid or Private Insurance but are still in need of services.

Our Role is Important, Our Results Matter

Central Montana Medical Center calls attention to the vital contributions to heath care made by CMMC’s medical laboratory science professionals. National publications have reported that up to 70% of the data used for clinical decision-making by physicians is generated by the clinical laboratory. Medical diagnosis and treatment decisions depend on timely and accurate test results. Working behind the scenes, laboratory professionals are a critical component of the health care team in patient care. The CMMC Laboratory offers testing for the purpose of screening, diagnosis, and monitoring of disease condition. Lab testing is performed on inpatients, outpatients, and referred samples, providing services 24 hours a day. CMMC laboratory now has the ability to perform five new tests that were previously referred to an offsite lab. Our clinical laboratory scientists are trained and deemed competent to perform all the testing that is performed on site. Only lower volume, highly specialized testing will now be referred out to other labs. The laboratory goes through a rigorous government inspection every two years to ensure compliance. What OB Services are available at CMMC?

CMMC has a labor and delivery, postpartum and nursery unit which is fully staffed 24 hours a day, 7 days a week. We are able to provide care to pregnant women, including those with prenatal complications (preterm labor, increased blood pressure, etc.), those having a vaginal delivery or scheduled or emergency c- sections. We provide postpartum care for new moms. CMMC has a Level I nursery, but do provide some services that Level II nursery's provide such as the ability to manage newborns with minor breathing problems, feeding difficulties, or newborns that may require IV antibiotics. This is done in consult with Neonatologists in Great Falls or Billings. Transport to those facilities is arranged if necessary.

What physicians in Lewistown provide OB services?

There are four Family Practice Physicians who provide OB services: Dr. Simpson., Dr. Marchion,  and Dr. Williams.  Dr. Williams is at One Health, phone # 535-6545.  Perinatologists from Billings and Great Falls are consulted regularly as deemed necessary by your primary physician.

Are childbirth classes available?

Prenatal Classes: Free of charge! and integrated into their prenatal appointments, scheduled through their primary physician office.  Their physician and clinic staff can help with answering questions and scheduling these visits.  Classes are taught by RN's and MD's with various guest speakers.   Drop In & Latch On Meetings:

  • Are you a breastfeeding mom?
  • Are you a mom with breastfeeding experience who would like to share her knowledge and expertise with other breastfeeding moms?
The Drop In and Latch On Group provides an opportunity to gather; with the guidance of a Certified Lactation Specialist, to share stories and offer support to each other in a warm, nurturing, and baby-friendly environment.  This group is very active on Facebook. Meetings are scheduled through the Facebook group. Contact the OB Department at 535-6286 Breastfeeding education and support is also available by Certified Lactation Consultants.

What is available for pain management during labor?

We encourage all  mothers-to-be to ask questions of their provider regarding pain management options during labor.  . Let your nurse know if you desire pain medication. All pain medications need a physician order so your nurse will need to contact your physician if you desire pain medications. Pain management can include pain medication given to you in your IV or intrathecal narcotic pain medication. An injection of a narcotic is done with an epidural. Nitrous oxide is also offered as an option for pain control. Ask your physician or nurse for more information.

What should I bring to the hospital?

Here is what we provide:
  • Large insulated mug for ice water
  • Toothbrush and toothpaste
  • Comb
  • Lotion
  • Kotex pads
  • Disposable stretch underwear to hold pads in place
  • Cleansing squirt bottle
  • Nightgowns & bathrobes while here
  • Diapers/clothes for baby while here
You will need to bring shampoo and conditioner, going home clothes for you and the baby, baby blankets, infant car seat and any personal items you may want. DON"T FORGET your camera. Remember though, no videotaping is allowed during the actual delivery (per hospital policy), however, you are more than welcome to tape before and after. Record those once in a lifetime moments!

Who can visit while we are in the hospital?

Father or support person may stay all night if desired. Hospital visiting hours are 11:00 a.m. to 8:00 p.m with exception during our "Shh...Nap Time" program. So our mom's can rest, we have limited visitation hours from 1:00 p.m to 3:00 p.m. each day. We require that all visitors check in at the OB Nurse's Station before visiting OB patients. We also ask that visitors not visit if they are ill or have ill children. Children may visit if they are not sick. The father of the baby or Mom's support person may visit at anytime. Father's may stay all night if desired. Check with the nurse.  The unit is locked at all times.  Visitors may be allowed access by the OB nurse if the visitor is on the approved visitor list given to staff by the patient.  Only those that the patient has specified will be allowed to visit in the OB Department.

What training/experience do the OB Staff have?

All are NRP (Neonatal Resuscitation Program) certified, Electronic Fetal Monitoring Certified, and have completed STABLE (Stabilization of newborn for transport) certification. All staff must also have CPR certification.

How do you provide security against infant abduction?

CMMC has an infant security system in operation at all times. Other security measures are also taken. Your nurse will visit with you about this when you are admitted.  The OB unit is completely locked from entry at all times, unless approved visitation from the patient.

Can I have my baby in my room with me all the time?

YES!!  Rooming-in is encouraged as a necessary step to bonding with your new baby as well as enabling you to be successful when you go home.  All services for your baby including weights, baths, and all tests well be done in your room with you there to comfort your baby.

What if I have questions about myself or my baby after I go home?

PADnet Testing The easiest and least expensive method to test for Peripheral Arterial Disease (PAD). 8-10 million Americans have PAD. Without proper treatment, 30% are likely to die in 5 years of PAD-related heart attack or stroke. PAD is a condition in which fatty deposits build up on the inner linings of arteries, and restrict the flow of blood to muscles and organs; especially the kidneys, stomach, arms, legs and feet. A PADnet test measures the blood volume in the patient’s extremities by using a series of blood pressure readings. This non-invasive test can be completed in 15-20 minutes at CMMC. This vascular test can also check for Peripheral Venous Disease (PVD) – a general term for damage, defects or blockage in the peripheral veins, most commonly found in the arms and legs (like varicose veins and chronic venous insufficiency).

Here at CMMC, we have partnered with Montana Pediatrics to bring you Montana-based care from dedicated pediatricians because we believe you deserve more options when it comes to accessing quality care after hours. By logging into a telemedicine visit, a Montana Pediatrics provider will be able to hear and see your child, understand your concerns, and treat your child from any phone, tablet, or computer with a camera. As soon as your visit is complete, Montana Pediatrics will make sure our clinic staff is notified of your visit. We will have your visit details and will reach out to you as soon as we are open again for any follow-up needs if necessary.

Central Montana Medical Center pharmacy provides medications for hospital patients being cared for in the emergency room, acute care, obstetrics, surgery department and infusion center.

The pharmacy is centrally located, distributing medications to the units for dispensing via automated medication cabinets. Sleep Study Sleep is not just a quality-of-life issue. Chronic sleep deprivation and poor sleep quality pose serious risks to your health. One of the biggest concerns is obstructive sleep apnea, which has been strongly linked with high blood pressure, coronary artery disease, congestive heart failure, stroke and heart attack. Apnea may also play a role in diabetes. In addition, people with sleep apnea are three to seven times more likely than others to be in a car crash. Same day surgery

Many procedures and surgeries are now done as "out-patient", in which patients can return to the comfort of their own home the same day.  Same day surgeries specialize in minimally invasive procedures, so recovery time and discomfort are minimal.  Typically, same day surgeries are scheduled in the morning to allow you to recover and return home later in the afternoon or evening. Our team of physicians, nurses and anesthetists monitor you throughout your surgical procedure and follow-up with you after your discharge from the hospital. Swing Bed Program

There is no place like home...

At Central Montana Medical Center, we are committed to helping you or a loved one recover here “at home” after surgery or hospitalization. Our swing bed program can provide a transition period between hospitalization and your return home. If you or a loved one are recovering from surgery, stroke, head injury, need IV antibiotics, or specialized wound care, our healthcare team here at CMMC is ready to help you recover while close to family and friends.

What is Swing Bed?
Swing Bed is a program established by Medicare specifically for smaller hospitals and communities.  It is the term used to describe a hospital room that can be used as an acute care bed and/or a skilled care bed.  The actual bed/room does not change; what changes is the level of care the patient receives. Not everyone is able or strong enough to return to everyday living after a hospital stay.  Some need extra time to recuperate and recover.  Swing Bed can help make a smooth transition between hospital and home. Even if you were hospitalized elsewhere, you can transfer to the Swing Bed program at CMMC bringing you closer to home and family.
Who qualifies for Swing Bed?
Patients who would benefit from CMMC’s Swing Bed Program are those who have already been in the hospital for at least 3 days, but require additional nursing care and/or rehab services on a daily basis. Some qualifications include:
  • Ongoing skilled nursing
  • Ongoing rehab services such as physical therapy, occupational therapy or speech therapy.
  • Long term inpatient IV antibiotic therapy
  • Specialized wound care
  • Accepted by CMMC and a local physician to the Swing Bed Program
Who pays for Swing Bed?
Medicare will generally pay for Swing Bed if the patient meets the following criteria:
  • Your Swing Bed care must be related to an acute hospital stay
  • You have a 3-night inpatient qualifying stay in the past 30 days
What can I expect while I’m a Swing Bed?
  • An individualized plan that is developed with you, your family and a multi-disciplinary team to establish and work toward discharge goals
  • Dress in your own clothes daily
  • Meet with therapist’s daily
  • Participate in activities as desired

TeleHealth CMMC is continuously innovating the way we deliver health care. We have several opportunities available for you to receive care online for minor medical issues as well as to continue your relationship with your primary and specialty care providers. Women’s Cancer Support Group

Meeting Times

Women's Cancer Support Group: meet the third Thursday of each month at Zion Lutheran Church from 12-1pm.   From coping with a diagnosis of cancer to handling treatment to dealing with long-term emotional and financial concerns, cancer patients and survivors often face stressful situations. Our goal is to help create a comfortable and relaxed environment for women to share their experiences and concerns. In addition to bringing in guest speakers and experts in fields such as nutrition, pain management, treatments and more, we will encourage an open discussion and dialog in a laid-back atmosphere designed to meet your needs. This group is a supportive network providing the necessary information to help everyone who has had or currently has any cancer diagnosis.

Wound Care

How do I know if I need Wound Care Services?

Anyone who has a non-healing wound may benefit from this specialty service.

What types of wounds can be treated?

  • Pressure ulcers
  • Venous stasis ulcers
  • Arterial ulcers
  • Chronic non-healing wounds
  • Non-healing surgical wounds
  • Radiation injuries
  • Ostomies (new and old)

What will I experience in the clinic?

Patients are generally referred by their primary physician, nurse practitioner, or physician assistant to the outpatient wound care department. You will then be evaluated by the wound care specialist where a complete review of your medicines, history, and evaluation of the wound will be done. There may be additional tests as necessary determined at that time. After the initial evaluation, the wound care specialist and your primary physician will develop a plan of care based on your individual needs. This involves regular visits to the outpatient wound care department where progression, evaluation, measurements, and treatment adjustments will be made if necessary to promote healing. The patient and family is always involved in the plan of care and instructed to help with the healing progression. This includes dressing changes, lifestyle, and diet. You will be taught to monitor progression between visits and to inform your primary physician and wound care specialist of any problems.

What kind of treatments may be used?

  • Wound VAC therapy
  • Specialized dressings - some have chemicals or medicines within fibers of the dressings that promote wound healing and limit the potential for infection. There are different types of dressings indicated for different types of wounds.
  • Special compression wraps - some wounds require compression to heal properly. Some diabetic wounds need special off-loading casts
  • Debridement - some wounds have unhealthy tissue which needs to be removed before healthy tissue can grow. Some wounds need surgical debridement which can be performed by our surgeon.
  • Nutrition - we also have a registered dietician and diabetic educator that you may see also as wounds need certain vitamins, minerals, and proteins order to heal properly.
  • Additional medical tests - you may need to have vascular studies, x-rays, or labs to see why a wound will not heal.
