About PatientConnect
With PatientConnect, you can:
- View and print your lab results, allergy list, immunization records, medication list, health issues, surgeries, and procedures.
- View and print your medical notes from your health care provider (for visits on/after 2/1/2020)
- View and print your radiology and lab results (for services on/after 2/1/2020)
- View your upcoming appointments
- Communicate with your doctor’s office by sending a secure message
- Request a prescription renewal through the secure message tool
You are responsible to protect the confidentiality of your username and password, as well as the health information you access using the portal. CMMC is not liable or responsible for misuse of your password or username. If you suspect that someone has learned your password, you should access the portal site immediately and change it. If you become aware of an issue or concern, for whatever reason, of this confidentiality, please promptly report it to the CMMC Privacy Officer at (406)535-7711.
CMMC reserves the right at our discretion to terminate patient portal offering, suspend user access or modify services offered through the patient portal.
