CMMC’s Care Coordination Dept provides an additional level of care for patients and their caregivers so that they can have an extra hand up in managing their chronic condition. We offer support wherever we can to make the transition into managing a long term health issue as helpful as possible. Central to our mission is the quality improvement initiative of incorporating community based care coordination designed to help patients achieve better health, better health care and lower costs.


 

Diadama Paden
Care Coordination Manager

Dama is our care coordination Manager and the Hospital/Clinic Social Worker. She has approximately 18 years of experience working in various fields of healthcare from NICU to Long Term Care. She studied at the University of Wyoming earning her Bachelor’s in Organizational Leadership. She joined our team in 2019 and specializes in working with families who’s loved ones are in the emergency room or on acute care. She assists with financial resources and is leading the Care Coordination department to serve our patients and help them to achieve their healthcare needs.

Eli Karinen, LCSW
Care Coordinator

Eli Karinen, LCSW provides individual and family therapy services to adults, adolescents, and children at Central Montana Medical Center. He studied at The Evergreen State College and the University of Montana where he earned his Masters of Social Work degree in 2013. He joined CMMC in 2017. He specializes in trauma, recovery, relationship issues, anxiety, depression, lifestyle and identity issues, and fitness and wellness. He provides a dynamic therapy experience rooted in evidence based practice with a client centered approach.
